3. Client Responsibilities
Clients are responsible for providing accurate information regarding their health history and current condition. You must inform us of any changes in your or your child’s health status, medical treatments, or circumstances that could affect the delivery of services.
4. Payment and Billing
A New Start ABA accepts various payment methods, including insurance, self-pay, and Medicaid. Payment is due at the time of service, and we will work with you to verify insurance coverage, when applicable. You are responsible for any fees not covered by your insurance plan or other payment arrangements.
5. Privacy and Confidentiality
We respect your privacy and are committed to protecting your personal information. All personal and health information provided to A New Start ABA is governed by our Privacy Policy and applicable state and federal regulations, including the Health Insurance Portability and Accountability Act (HIPAA). Either party may terminate services at any time. If you wish to discontinue services, please notify us in writing. A New Start ABA may terminate services if there is a violation of these Terms, failure to pay for services, or if the continuation of services is not in the best interest of the client.
